What the ASCeXAM is - and the clock that shapes everything

The ASCeXAM - formally the Examination of Special Competence in Adult Echocardiography - is the National Board of Echocardiography's exam for physicians who want to demonstrate competence across all of adult echo. It's the credential exam for cardiologists reading echo, and it's computer-based, administered at Pearson VUE centers with a remote OnVUE option for those who'd rather test from home.

The single most important logistical fact: it runs once a year. Miss the registration window and you're not studying harder - you're waiting twelve months. To sit the exam you need proof you can practice medicine - a valid license or a letter of good standing from an ACGME program director; certification documentation is not required at registration and can be submitted any time after. Which brings up the distinction that confuses every first-time candidate: passing makes you a Testamur, not board certified. One more reason cardiologists take this exam seriously: passing earns ABIM MOC points. The NBE reports passers to ABIM monthly, and points apply for exams passed after July 2014 - so the same effort feeds both credentials.

What the exam covers - and how it's really weighted

The blueprint spans the whole discipline:

  • Ultrasound physics and instrumentation - including the artifact questions that decide more scores than any other single topic
  • Valvular heart disease: identification and, critically, quantification of severity
  • Systolic and diastolic function, including strain imaging and 3D echocardiography
  • Contrast echocardiography
  • Ischemic heart disease and stress echo
  • Congenital heart disease in the adult
  • Doppler hemodynamics: gradients, valve areas, shunts, pressure estimates

And the same unofficial truth that governs every NBE exam: it's image-heavy. Candidates consistently describe the pass as being decided at the loops - recognizing which measurements are valid on this image, catching the artifact masquerading as pathology - rather than at the textbook. Layer 1 - A structured review

One comprehensive source covering the blueprint. The institutional standard is the ASE's ASCeXAM Review Course - virtual, faculty-led, spanning physics through congenital disease, and carrying 28.75 AMA PRA Category 1 credits, which conveniently covers the certification application's entire CME requirement in one purchase. Alternatives with the same job: EchoSAP (the ACC's self-assessment program, aligned to the ASCeXAM blueprint, with ABIM MOC points built in) or a standard board-review text.

Layer 2 - A question bank

Exam-format questions with explanations, worked until your percentages stop embarrassing you. The ASE's Online Practice Exam Simulation is the closest thing to the real event - a 300-question bank with 400+ images and video, structured as two 150-question simulated exams you can repeat up to six times across an eighteen-week access window. Layer 3 - Daily image interpretation (the layer most people skip)

A month-by-month plan (six months out)

2 before Timed question blocks. First full pass of the ASE practice simulation. Revisit error-log topics. Start committing to reads out loud before checking answers - the discipline of deciding is the exam skill.
1 before Second simulation pass and timed blocks. Hemodynamic-calculation drills until automatic. Physics formula sheet weekly, not nightly - sleep outperforms cramming on image-recognition exams.

After you pass: Testamur status starts the clock on the certification application - 15 echo-specific CME credits, case documentation, and a Certification Committee that reviews applications only twice a year, so assemble the paperwork early. Is the ASCeXAM hard?

Fair but wide. Its difficulty is breadth - physics through congenital disease - rather than trickery, and the candidates who struggle almost always share the same gap: strong reading, thin image reps. Regular interpretation practice is the difference between an anxious pass and a comfortable one.

ASCeXAM or PTEeXAM - which one do I need?

Different exams for different practices: the ASCeXAM covers all of adult echocardiography and is the cardiologist's credential; the Advanced PTEeXAM covers perioperative TEE and is the cardiac anesthesiologist's.
